FAMILY PRACTICE
FULL-SERVICE FAMILY PRACTICE, ALL AGE GROUPS,
WITH CHRONIC CARE MANAGEMENT, AND CARE OF ACUTE ILLNESS
USING A UNIQUE BLEND OF INTEGRATIVE MEDICINE AND SCIENCE
AS WELL AS CRYOTHERAPY, SKIN BIOPSIES, LACERATION REPAIR, AND OTHER MINOR PROCEDURES.
Direct Primary Care
A simple and direct relationship with a practice that has your needs as central and answers to you, the patient, without intermediaries. We want to bring back the primacy of the doctor patient relationship. Welcome.
As of Jan 1, 2026, HSA funds can be used tax free to pay for DPC membership fees, with a limit of $150/person, $300/family. Pls see One Big Beautiful Bill Act - H.R.1 for details.
Family/Household

Family Rate: $300 per month for two parents + 2 children up to/including age 18, ($50 per per child per month for each additional child).
Single Patient

-Home visits are available if deemed appropriate by the physician and based on physician availability. An extra charge will apply: $50 dollars for under a half hour round trip travel and $100 for over a half hour hour of round trip travel.
​-For those utilizing care more than 10 visits yearly, or in either assisted living or hospice, a high touch membership fee may be activated at $200/mo.
​-Procedures will be billed a low fee for materials (lacerations, lesion removal, cryotherapy of warts and actinic keratoses, and joint injections with steroids).
-Payment is expected monthly. Accounts that are not current are subject to review. While we are understanding of individual circumstances, we will be applying consistent expectations to all.

Three month minimum membership.
No show charge of $50 will be applied for cancellation less than 24 hours prior to apt or to no shows. Pls call 802 453-4696 to both schedule and cancel appointments.

Transparent Fee & Payment Terms

NO VISIT FEES.
Membership fees will cover doctor visits, virtual consultations, and coordination with consultants.
Labs and imaging will be handled through insurance, if applicable.
Some drugs can also be dispensed at the practice, which can amount to significant savings. Please let the office know if your drug prices are beyond your means.
